CoRe Order Form – Self Calculating(to be mailed)

The new order form can be used two ways; you can print it and fill it out by hand, including the calculations…OR you can open it on your computer as a PDF and fill it in.  It will automatically add any necessary taxes and total your order.  Then, all you have to do is print and fax or postal mail to CoRe.  Fax number and address for CoRe is on the order form.

Enter number of books for order in the first row, it will automatically calculate the cost savings based on number of books ordered.

Things To Remember

You cannot save the file [with the reader] – you must print it.

You must fill in the name, address, city, state and zip; if the “ship to” is the same as the billing address, just type SAME in the name field of the billing address.

Mail only the billing info page and the order page (pages 1 and 2).

The new order form can also be emailed after filling in data! To do this you must have Adobe Acrobat [not the reader] or ability to scan the printed document.

If you have full Adobe Acrobat, open up the PDF form, fill it out and then click on the 'file' menu and then 'send as an email.'  This will create an email if you use Outlook or some other form of email program.  If you use a web-based email program, you may have to save the form and attach it manually.  Email to coreorders@coda.org.

If you have the ability to scan the document, fill it out, print it and then scan it.  Manually attach your scan to an email and sent it to coreorders@coda.org.
